In need of care tip, for a very anxious dog: what have been some methods you use to help deal with that?

Just GAD in a dog. We've been working one-to-one with a trainer, but our dog still has moments where he goes crazy at a knock on the door, or from someone coming or going, or he won't charge the fence at the neighbor dog once moment but he will the next. Don't know if it's a protection thing or something else.
